♦ Find the letter S for Paul Newell Seaman LCSW and click it
♦ Scroll to the name Paul Newell Seaman LCSW in the list and click
♦ On the top left find "I'm new here Help Me" and select (click) "Set My First Appointment"
♦ Please fill in all of the form
After filling in this Basic Information you will be asked to fill in a brief Biographical Information Form
before our first appointment. It will take about 20-30 minutes to complete and must be done all at once.
If you have time to complete it now, please do so. If not, remember to log in again before our
first appointment and complete the form at that time.
If you are planning on using insurance or another form of payment other than yourself please realize
that there are a wide variety of insurance policies and coverages. If you want your first appointment to be
covered by insurance please schedule your appointment several days from today so that I will have time
to verify your coverage or discuss my financial policies with you. You are responsible to know
about your coverage (deductable, etc.) so please check this before our first meeting. Ultimately
settlement of the fee for your service is your responsibility.
I strongly recommend you fill the forms out online, however, if you wish to complete the forms by hand
please see the "Helpful Forms" page on this website. Download them to your local printer, fill them out,
and bring them to our first appointment . If you are using insurance please bring a copy of the front and back
of your insurance card.
