If you are a first-time patient, please go to the previous page on this website
called "Appointment Request" and follow the directions.  If however you do not wish
to fill out forms online, please:
1. Select the " New Client Intake" "Policies" and "Limits of Confidentiality"  forms from the bottom of this page.
2. Download (Print ) them out locally to your printer.
3. Review, complet)e, sign, and bring them to our first appointment.
4. If you are using an insurance plan, please bring a photocopy of the
     front and back of your insurance card.

If you would like me to coordinate care with another provider (for example,
your psychiatrist, endocrinologist, etc.), please complete the
Authorization for Release of Confidential Information
